- Input Your Phone Number: Enter your phone number and select the network provider (MTN, Glo, Zain, or 9mobile).
- Specify Your Phone Type: Indicate whether you own a smartphone or not.
- Select Your State and Local Government Area: Choose the state you reside in and the correct local government area.
- Provide Ward Information: Fill in the name of your ward in the designated field.
- Commit to Physical Screening: Acknowledge that you will attend a one-day meeting for physical screening in your community to be certified as a trained volunteer.
